Aging Services

JFS Rochester understands the unique needs and challenges faced by older adults, and we are committed to providing high-quality aging services that meet those needs. We work alongside JFS clients and their families to develop a plan of care that ensures opportunities for self-sufficiency, support, and dignity through healthcare, mental health, nursing, transportation, and safety supports.

About Our Programs

Our comprehensive care management program is designed to provide older adults with the support and resources they need to age in the place of their choosing and maintain their independence.

Our experienced care managers work with older adults and their families to develop personalized care plans that address their physical, emotional, and social needs.

JFS Rochester’s support groups for caregivers provide a safe space for individuals to share their experiences and offer support and encouragement to one another. We also provide access to a range of essential services and emergency supports, that help older adults maintain their quality of life.  We understand that aging may bring with it strong feelings of grief, loss, and anxiety, and our mental health counselors are prepared to help older adults face these feelings and move forward to a place of peace and resolve.

Aging Counseling

The Counseling Center at JFS offers services for aging adults and their caregivers to address the unique challenges that come with aging and caring for older loved ones. Our experienced therapists work with older adults and their caregivers to improve communication, navigate complex medical and social systems, and promote emotional wellbeing and quality of life for all involved.

Caregiver Support

Part of JFS Rochester’s commitment to our area’s aging population is offering support for caregivers, helping to reduce stress, provide safe and appropriate care, alleviate ongoing or emergency financial burden, and provide a sense of community.

Connections for Older Adults

The Friendly Connections and JFS Connects programs both support ongoing connections for Rochester’s older adults to help them avoid the damaging effects of issolation and play an active role in our community. Friendly Connections connects older adults with volunteers that offer a check-in, and provide comfort and companionship. JFS Connects joins seniors with new friends in our community and beyond via a Virtual Senior Center.

EISEP Aging Care Management

Expanded In-home Services for the Elderly Program (EISEP) is a state and county funded service that provides care management, subsidized home care, and other services to help older adults stay independent in the community and delay higher levels of care that require Medicaid support.

Rochester Highlands

JFS Rochester delivers holistic programming and resource connections to residents at The Rochester Highlands, a local affordable housing complex. Programming for residents includes care management, nursing services, educational workshops, health and wellness offerings, and child-focused activities.

The NORC at Ellison Park

The JFS Rochester NORC (Naturally Occurring Retirement Community) program uses an innovative model for aging in place in your own home. NORC seniors continue to grow and learn, maintain their independence, and enhance their quality of life by making new friends and actively participating in their community.
