JFS Aging Care Management

JFS Aging Care Management is a program that provides older adults with comprehensive care management services to help them navigate the challenges of aging. Our care managers are dedicated to providing excellent service delivery and care coordination that maintains dignity and achieves the goals of the older adults we serve.

Our care managers work closely with older adults and their families to develop customized care plans that address their unique needs and preferences. We provide support and resources to help older adults live safely and independently in their homes for as long as possible. Our care managers can assist with a wide range of needs, including managing chronic health conditions, coordinating medical appointments, arranging for home health care services, and connecting older adults with community resources.

In addition to providing support to older adults, our care managers also work closely with family members and caregivers to ensure that they have the information and resources they need to support their loved one’s needs. We understand that caring for an older adult can be challenging, and our care management services are designed to help support caregivers in tandem with our support of the older adult.
