EISEP Aging Care Management

About the Program

The EISEP Aging Care Management program is a state and county funded service that provides care management, subsidized home care, and other services to help older adults stay independent in the community and delay higher levels of care that require Medicaid support. At JFS Rochester, our care managers work closely with older adults to develop individualized care plans that meet their unique needs and preferences.

Through the EISEP program, older adults have access to a range of services that can help them maintain their independence and quality of life. These services may include assistance with activities of daily living, such as bathing, dressing, and meal preparation, as well as home health care services, transportation assistance, and personal emergency response systems. Our care managers also work closely with family members and caregivers to provide them with the information and resources they need to support their loved one’s needs. We understand that caring for an older adult can be challenging, and our care management services are designed to help alleviate some of the stress and burden that caregivers may experience.
